Any pupil entering this school should present a certification from a licensed physician that he or she has received a test for tuberculosis and has received or is in the process of receiving adequate immunization against poliomyelitis, diphtheria, pertussis, rubeola, rubella, mumps, and tetanus according to the recommendations of the State Department of Health.
Anyone transferring immunizations will have 60 days to do so. A copy of a birth certificate and social security number is also required.
